Community Housing Network develops, owns and manages permanent supportive housing for people challenged with mental health conditions, addiction disorders and histories of homelessness in Central Ohio. By collaborating with partners to provide linkages to supportive services, CHN expands opportunities for its residents to successfully maintain housing stability. CHN’s 24/7 Supportive Housing provides linkages to services and on-site monitoring 24 hours a day, seven days a week for individuals with compounding disabilities. Service-Enriched Housing provides on-site resident management and linkages to social, health and employment services. CHN’s Independent Housing provides affordable housing through rent subsidies for low-income persons with compounding special needs, and is also linked to support services. Success is measured by length of stay in housing, positive exits from housing, use of supportive services, as needed, and increased income.

Annually, Concord helps over 4,000 friends, co-workers, neighbors, and family members improve their emotional health so they can successfully deal with the challenges we all face and live more successful lives. Concord is a not-for-profit community mental health organization serving our community for over 46 years. We believe and live our mission of Instilling Hope and and Serving with Heart. Concord’s programs include Prevention Services in schools to help keep children in school, Outpatient Counseling for all ages to help adults, families and the elderly live fulfilling lives and reach their full potential; Special Services to the Elderly and for people who suffer from Severe Mental Illness so that both groups can live safely and independently; and services for Substance Abuse including both Outpatient Counseling and our Intensive Outpatient Program to help our neighbors who are overcoming addictions. Founded in 1961 with a Christian commitment to service, National Church Residences is a not-for-profit organization driven by a mission to provide quality residential communities for all seniors, in addition to high quality care and services enabling seniors to remain home for life. National Church Residences serves more than 42,000 seniors through housing and health care services. With 340 communities in 26 states and Puerto Rico, it is the nation’s largest not-for-profit provider of affordable senior housing and the largest manager of service coordinators. It also offers senior residential communities, home and community-based services, and permanent supportive housing.

North Community Counseling Centers is a non-profit, community mental health agency that provides mental health, case management, psychiatric, and addiction treatment services. With four locations, home, and school based programs, NCCC is able to reach out to individuals, couples and children throughout Franklin County who are struggling with challenges related to mental illness and addiction. NCCC provides individual and group counseling for mental health and substance use disorders as well as school-based services. We provide case management services to assist in eliminating environmental barriers and linking with resources for a higher quality of life, including housing coordination. We provide psychiatry, medication management, and medication assisted substance use treatment. Our services provide culturally-centered treatment for all of our clients and have partnered with leaders in the Bhutanese-Nepali community to address the needs of this ever-growing population.

Southeast, Inc. is a comprehensive provider of community mental health, primary health care, dental services, addiction treatment services, vocational services, housing and homeless services that began operations in Franklin County (Columbus, Ohio) in 1978 and serves over 15,000 people in eight counties in Ohio.
Southeast also operates Friends of the Homeless and the Fresh A.I.R. Gallery.
Southeast provides healthcare for people who are homeless, living in transitional housing, or have been previously homeless. Southeast coordinates and integrates care across services for all clients across all their healthcare locations. Southeast is a Federally Qualified Health Center and also offers individual, couples, kids and family counseling and outpatient chemical dependency services. Southeast also offers community based services, case management and psychiatric services and support for persons with severe and persistent mental illness with specialty treatment teams.
