CompDrug has an immediate opportunity for a Case Manager to provide community resource linkage and crisis management. CompDrug’s approach is trauma-informed; the successful candidate will have a strong boundary system coupled with a high level of respect and empathy.
Work Arrangements:
Full Time, 40 hours per week. Hours typically fall between 5:45 am and 6 pm. Work will be conducted in person,

Work Location: 50% onsite & in field/50% remote
Reports to: Manager of Clinical Services
Essential Duties/Functions include:
Assessing clients’ needs, which may include biopsychosocial, preferences and abilities and developing plans to improve their conditions
Working closely with other staff members to personalize the client care
Coordinating and providing care that is safe, timely, effective, efficient, equitable, and client centered
Listening to client’s concerns and providing interventions as required
Monitoring cases by advocating for needed services and resources; obtain additional resources; intervene in crises; provide personal support.
Maintains clients’ records by reviewing case notes; logging events, recording actions taken, referrals made and progress the client has made
Communicates clients’ progress by disseminating results and obstacles to other professionals and identifying treatment influences.
Handling case assignments, treatment plans, review progress.
Help clients achieve wellness and autonomy.
Develops a database of services available to the underserved- maintains a robust directory of service providers for use throughout the clinical department.
Liaison with outside agencies that meet case management services.
Meets clients in the field to facilitate connection with services and resources.
Other duties as assigned.
Regular and timely attendance.
Assist with the NPI process and establishment and maintenance of CAQH profile, if requested.
