Who We Are
CHN develops and manages affordable permanent supportive housing for vulnerable people experiencing mental illness, substance use disorder, trauma-related issues or homelessness. CHN collaborates with its network of partners, which includes human services, healthcare, counseling and other experts throughout Franklin County, to offer residents access to services like service coordination with case management, crisis intervention, healthcare, counseling, food access, employment training, benefit assistance and more.
Housing coordination
Our services are offered for our residents who are referred from the Community Shelter Board and ADAMH.
