Your Job Opportunity
In this role, you will provide high-quality counseling services consistent with professional standards, best practices and Concord’s policies to older adults (60 and over), focusing on supporting their independence and promoting good mental health.
•Deliver individual therapy to clients with mental health diagnoses and/or substance use disorders
•Conduct comprehensive assessments and develop individualized treatment plans
•Apply clinical expertise within a multidisciplinary care model
•Collaborate with providers and others as needed to support client success
•Assess risk and respond to crisis situations during office hours
•Provide services in-person, via telehealth, and in the client’s home, based on client needs
•Maintain accurate, timely clinical documentation in an electronic medical record system
Qualifications Needed for Success for a Therapist for Older Adults
•Master’s degree in social work with independent licensure: LISW/LPCC/LMFT required
•Enrolled on Medicare, Medicare Advantage, and/or private insurance panels is preferred.
•Ability to maintain thorough, timely, well documented treatment plans, case notes and intake summaries,
illustrating caseload measurable treatment progress in computerized medical records system.
•Must have working knowledge of mental illness, psychiatric symptoms, and their impact on functioning and behavior working with older adults and understanding their unique challenges.
•Ability to pass a pre-employment background check and drug screen.
