Case management/ care coordination
COAAA helps older adults and individuals with disabilities live independently and safely in their homes for as long as possible.
COAAA provides case management for a number of long-term care services. These services give individuals – who meet eligibility criteria – choice and dignity to live independently. Whether the need is personal care services, homemaking, transportation or home-delivered meals, COAAA can coordinate a care plan to meet the person’s needs.
COAAA also helps family caregivers. COAAA provides free consultation, education, and support to help caregivers deal with the complex issues of caregiving. Staff can advise on hiring in home help, provide referrals to financial assistance programs and provide referrals to caregiver respite services.
COAAA serves eight central Ohio counties: Delaware, Fairfield, Fayette, Franklin, Licking, Madison, Pickaway and Union.
